How to Fill out a Form in GoFormz

1. Go to https://www.goformz.com/ and login with your username and password for your area (username ex: martinez.area@teammurph.net) or you can use the GoFormz App on your phone.

2. After you have logged in choose the Forms section at the top of the screen to select the form you need. 

3. After the Forms page is open go the upper right and choose New. 

4. Then you will have to find the form you are looking for in the folders provided. For this article we are using the 5 Star Opening Checklist found in the Dominos folder.

5. Once the form is selected you can start filling out the highlighted areas in the form.

6. Once the form is filled out you will click the Complete button in the top right corner and your form will be submitted.
