Product Shipping Kit Procedure

•Remove the new Product Incident Kit from the Shipping Kit and keep it at your store for future use.

•Write the Claim number on the Domino's Sample Analysis Request Form (included for alleged illness samples only). Place this form in the document (small) pouch of the Product Incident Kit along with the Product Complaint Form.
•Place the Product Incident Kit containing the sample and paperwork in the styrofoam cooler with the ice packs.
•Replace the lid on the cooler and tape the outer box shut.
•Detach the shipping label from the customer receipt along the dotted lines. Keep this receipt for your records.
•Peel the self-adhesive shipping label from the backing and attach the label to the box top.
•Alleged Illness samples may be shipped on Friday, however, the Saturday Delivery label must be placed on the outside of the box.
•Do NOT ship other types of samples on Friday.
•Call the 800 number on the shipping label to arrange for sample pickup.
